Understanding complex and overlapping neurodevelopmental needs

Some children and young people experience difficulties that are layered, inconsistent and difficult for others to fully understand. Families are often told that their child is “too complex”, “doesn’t fit neatly into one category”, or that different professionals see different parts of the picture.

Children with Fetal Alcohol Spectrum Disorder (FASD), prenatal alcohol exposure, developmental trauma, sensory processing differences and overlapping neurodevelopmental profiles may experience significant difficulties with regulation, participation, emotional wellbeing, executive functioning and everyday life.

At Activate OT Scotland, we provide neurodiversity-affirming Occupational Therapy assessment and support for children and young people with complex neurodevelopmental presentations, including FASD and overlapping sensory, regulation and participation difficulties.

Our approach focuses on understanding the whole child — not just isolated behaviours, diagnoses or difficulties.

Children and young people may:

  • become emotionally overwhelmed very quickly

Understanding complex neurodevelopmental profiles

Children with FASD and other complex neurodevelopmental presentations often experience overlapping differences affecting:

  • sensory processing and regulation

  • executive functioning

  • emotional regulation

  • motor coordination and planning

  • attention and participation

  • interoception and body awareness

  • transitions and adaptability

  • fatigue and nervous system stress

  • school participation and attendance

These presentations are frequently misunderstood because difficulties may vary significantly depending on the environment, demands, stress levels and support available.

A formulation-led approach

At Activate OT Scotland, we use a formulation-led approach to understand how sensory, emotional, cognitive, developmental and environmental factors interact to affect participation and everyday functioning.

Rather than focusing only on labels or behaviours, we aim to understand:

  • why certain environments or demands are difficult to sustain

  • how sensory and nervous system differences affect participation

  • why functioning may appear inconsistent

  • what supports may help children feel safer, more regulated and more able to participate in everyday life

This approach is particularly important for children with overlapping or complex presentations that may not fit neatly into one diagnostic category.
