FASD & Complex Neurodevelopmental Profiles
Understanding complex and overlapping neurodevelopmental needs
Some children and young people experience difficulties that are layered, inconsistent and difficult for others to fully understand. Families are often told that their child is “too complex”, “doesn’t fit neatly into one category”, or that different professionals see different parts of the picture.
Children with Fetal Alcohol Spectrum Disorder (FASD), prenatal alcohol exposure, developmental trauma, sensory processing differences and overlapping neurodevelopmental profiles may experience significant difficulties with regulation, participation, emotional wellbeing, executive functioning and everyday life.
At Activate OT Scotland, we provide neurodiversity-affirming Occupational Therapy assessment and support for children and young people with complex neurodevelopmental presentations, including FASD and overlapping sensory, regulation and participation difficulties.
Our approach focuses on understanding the whole child — not just isolated behaviours, diagnoses or difficulties.
Children and young people may:
become emotionally overwhelmed very quickly
struggle with unpredictability, transitions or changes in routine
experience frequent meltdowns, shutdowns or emotional dysregulation
mask difficulties in some environments but collapse in others
struggle with attention, impulsivity or executive functioning
appear developmentally inconsistent across different skills
experience sensory overwhelm or sensory seeking behaviours
struggle with participation at school, home or in the community
become exhausted by everyday demands
experience difficulties with organisation, memory or task completion
appear anxious, avoidant or oppositional when overwhelmed
struggle to maintain friendships, routines or independence
Many children work extremely hard to cope and may appear more capable than they actually feel internally.
Understanding complex neurodevelopmental profiles
Children with FASD and other complex neurodevelopmental presentations often experience overlapping differences affecting:
sensory processing and regulation
executive functioning
emotional regulation
motor coordination and planning
attention and participation
interoception and body awareness
transitions and adaptability
fatigue and nervous system stress
school participation and attendance
confidence, relationships and wellbeing
These presentations are frequently misunderstood because difficulties may vary significantly depending on the environment, demands, stress levels and support available.
Many children have experienced repeated frustration, overwhelm, exclusion or misunderstanding before families seek specialist support.
A formulation-led approach
At Activate OT Scotland, we use a formulation-led approach to understand how sensory, emotional, cognitive, developmental and environmental factors interact to affect participation and everyday functioning.
Rather than focusing only on labels or behaviours, we aim to understand:
what may be contributing to overwhelm
why certain environments or demands are difficult to sustain
how sensory and nervous system differences affect participation
why functioning may appear inconsistent
what supports may help children feel safer, more regulated and more able to participate in everyday life
This approach is particularly important for children with overlapping or complex presentations that may not fit neatly into one diagnostic category.
How Occupational Therapy can help
Occupational Therapy can help families and schools better understand the factors affecting participation, regulation and everyday functioning.
Assessment and support may include:
sensory integration-informed assessment
understanding sensory processing and regulation needs
executive functioning and participation assessment
exploring environmental stressors and participation demands
practical strategies for home, school and community life
emotional regulation support
recommendations for reducing overwhelm and fatigue
school collaboration and functional recommendations
support for transitions, routines and everyday independence
detailed formulation and professional reporting for complex presentations
Our aim is to support children and young people in experiencing greater understanding, regulation, participation and wellbeing across everyday life.
Assessment and support pathways
Some children benefit from focused assessment and practical recommendations, while others may require more comprehensive assessment to fully understand the interaction between sensory processing, regulation, participation, executive functioning and environmental demands.
Depending on your child’s needs, support may include:
Standard Assessment
Enhanced Assessment
Comprehensive Assessment
School Observation & Professional Reports
Related Areas of Support
Complex neurodevelopmental profiles may overlap with:
sensory overwhelm and regulation difficulties
autism and ADHD
dyspraxia/DCD
school distress and burnout
emotional regulation difficulties
participation fatigue and exhaustion

Next Steps
If your child’s difficulties feel complex, inconsistent or difficult to fully understand, an initial parent consultation can help us explore your concerns and consider whether Occupational Therapy assessment or support may be helpful.
